CONDITIONS OF ATTENDANCE

Liability
I understand that I am registering for instruction in an activity which may involve physical, emotional, and/or psychological activity and that in case of such an undertaking there is always a risk involved. I understand that in a group setting the instructor cannot always monitor my physical, emotional, and psychological limits, and that I am therefore responsible for assessing the risk any activity poses to me, and choosing a safe course of action for myself.
I agree to assume the risk and responsibility for any injuries or damages suffered by me arising out of my participation in this workshop.

Food Allergies
In connection with any food allergies I have, I understand that I am responsible for knowing whether any ingredients to which I am allergic exist in any foods that are prepared by Mindful Health, LLC. If I am uncertain about the ingredients of these foods, I am responsible for learning that information from the senior people in the café or restaurant who have prepared or overseen the preparation of such food.

Use of Likeness and Photography
I understand that while I am on a Mindful Health retreat, attending a Mindful Health conference, or participating in any other Mindful Health event, Mindful Health may be taking photographs of participants. Some of these photographs will be used by Mindful Health for promotional, educational, or informational purposes in its catalogs, brochures, website and newsletters. By attending a Mindful Health retreat, or participating in any other Mindful Health event, I agree that Mindful Health may take photographs that include my image and that such photographs may be used by Mindful Health for promotional, educational, and informational purposes without compensation to me of any kind.

Recording Devices
The use of recording devices is strictly forbidden during retreats, conferences, keynote speeches, events, or any other programs with Mindful Health or at such programs sponsored or presented by Mindful Health. CDs and DVDs of many workshops may be made available through Mindful Health in the future.

Privacy Notice
Mindful Health knows that you care how information about you is used and shared, and we appreciate your trust that we will do so carefully and sensibly. This notice describes our privacy policy. By visiting Mindful Health's websites, you are accepting the practices described in this Privacy Notice.

Personal Information Mindful Health Gathers
The information we receive from you helps us personalize and continually improve your Mindful Health experience. Here are the types of information we gather.

• Information You Give Us: We receive and store any information you enter on our Websites or give us in any other way. You can choose not to provide certain information, but then you might not be able to take advantage of many of our features. We use the information that you provide for such purposes as sending you catalogs, customizing your Mindful Health experience, improving our customer service, and communicating with you.
• Automatic Information: We receive and store certain types of information whenever you interact with us. Mindful Health's Websites uses temporary cookies. Cookies are pieces of information that a website sends to your computer while you are viewing the website. These pieces of information allow the website to remember important information that will make your use of the website more useful. The visitor can alter web browser settings to turn off cookies or allow the computer to display a warning each time a cookie is being received.
• Information from Other Sources: We might receive information about you from other sources and add it to our account information.

Security

• Secure Sockets Layer: To protect the security of your information while you are visiting Mindful Health's websites, Secure Sockets Layer (SSL) software that Mindful Health uses encrypts the personal identifiable information you input.
• Login Profile: Some of Mindful Health's Websites may provide you with the opportunity to create a login profile that allows you to save your contact and other information for speedier registration. We recommend you keep this information current. Mindful Health is not responsible for updating login profile information.
• Passwords: To provide you with an increased level of security, access to your login profile is protected with a password you select. We strongly recommend that you do not disclose your password to anyone. Your password will never be asked through unsolicited communication from Mindful Health.
• Third-Party Websites: Mindful Health's Websites may sometimes include third-party advertising and links to other Websites. We do not provide any personally identifiable customer information to these advertisers or third-party Websites. Mindful Health has no control over the privacy practices of sites outside our domain. If you follow a link to another Web site, please read that site's privacy policy.

Credit Card Transactions

• Secure Sockets Layer: To protect the security of your information while you are visiting Mindful Health's websites, Secure Sockets Layer (SSL) software that Mindful Health uses encrypts the personal identifiable information you input.
• Minors: Minors (anyone under the age of 18) are not eligible to pay for Mindful Health services and we ask that minors do not submit credit card information. Mindful Health is not responsible for a minor's use of their parent's credit card.
• Incorrect or Fraudulent Information: A credit card transaction may be canceled by Mindful Health if we reasonably determine that the order information you provided is incorrect or fraudulent, or if we reasonably believe the credit card utilized is from an unauthorized party.

• Promotional eMails: If you do not wish to receive promotional emails from Mindful Health such as our email newsletter, please follow the instructions at the end of emails you receive from us. From these emails you can 'unsubscribe' or 'manage your subscription preferences'.

Privacy Policy Changes
This Privacy Notice and Privacy Policy was posted on August 10, 2012. Mindful Health reserves the right to change this Privacy Notice and Privacy Policy without notice.

Cancellation Policy
Mindful Health reserves the right to cancel a program at any time. If Mindful Health cancels a program, you will receive a full refund.

A full refund, less a $500 processing fee, will be offered for cancellations made 90 days before the retreat start date. No refund or credit will be offered for cancellations made with less than 45 days notice; if you do not show up; or if you leave a program early.
